Continuous Intraoperative Recurrent Laryngeal Nerve Monitoring: Techniques, Applications, and Controversies

چکیده

Abstract Purpose of Review the present review is to illustrate current state art concerning continuous intraoperative recurrent laryngeal nerve monitoring (cIONM) for thyroid surgery. Recent Findings cIONM potentially leads an improved postoperative vocal cord palsy rate, compared intermittent technique. There are currently two main approaches cIONM: either conventional based on vagal stimulation or experimental methods, which do not require positioning a electrode. One these methods recently described technique “LAR-cIONM,” utilizes adductor reflex. Summary represents advancement monitoring, allows immediate reaction signal changes. Threshold values and guidelines prevent were validated direct verification alterative including LAR-cIONM.

Intraoperative laryngeal nerve monitoring during thyroidectomy.

OBJECTIVE To determine whether nerve integrity monitor testing during thyroidectomy predicts recurrent laryngeal nerve (RLN) function after surgery. DESIGN Prospective cohort outcomes study PATIENTS The study included 210 consecutive patients with thyroid abnormalities who underwent thyroidectomy. METHODS All patients were intraoperatively monitored with a nerve integrity monitoring syste...

Continuous intraoperative vagus nerve stimulation for identification of imminent recurrent laryngeal nerve injury.

BACKGROUND Conventional intraoperative nerve monitoring, predicated on intermittent stimulation, can predict recurrent laryngeal nerve (RLN) palsy only after the damage has been done. METHODS Fifty-two patients (52 nerves at risk) who underwent continuous intraoperative nerve monitoring (CIONM) for thyroid surgery via vagus nerve stimulation had their electromyographic (EMG) tracings recorded...

Role of Intraoperative Nerve Monitoring During Parathyroidectomy to Prevent Recurrent Laryngeal Nerve Injury

Injury to the recurrent laryngeal nerve (RLN) is a well known, though less frequent, complication of parathyroid surgery. In recent years, the use of intraoperative nerve monitoring (IONM) has gained popularity amongst surgeons when operating on thyroid gland; however, its utilization in parathyroid surgery is not established. This trend continues to rise, despite multiple studies documenting n...

Meta-analysis of recurrent laryngeal nerve injury in thyroid surgery with or without intraoperative nerve monitoring

Intraoperative nerve monitoring (IONM) aimed at reducing the injuries of recurrent laryngeal nerve during thyroidectomy is controversial. We conducted a meta-analysis to assess the incidence of nerve injuries with or without IONM. Studies published from January 1994 to February 2012 in English language on humans were identified. Heterogeneity of studies was checked by the Higgins test. Summary ...
